Australian Primary Care Collaboratives (APCC)
The Australian Primary Care Collaboratives program (APCC) supports general practice to make changes in the management of patients with diabetes and coronary heart disease, as well as improving patient access. The three key factors underpinning the success of the collaborative methodology are:
- The rapid improvement cycles known as the Plan-Do-Study-Act (PDSA) cycles,
- Monitoring progress through monthly data measurement, and
- The sharing with and learning from others.
Participating practices then help ‘spread’ the collaborative methodology by offering their support and knowledge to practices new to the APCC program and into the wider health community.
Phase 1
Across Australia approximately 600 practices participated in Phase 1 of the program, including 19 (22%) practices in the former Perth and Hills Division area of Perth Primary Care Network (PPCN).
Phase 2
PPCN currently has three practices participating in Phase 2 of this program and will be recruiting further practices for a local wave in 2009.
