GP – Hospital Liaison
GP – Hospital Liaison staff provide a vital link between Hospitals, Divisions of General Practice and General Practitioners to improve continuum of patient care through improved communication. The broad goals of Hospital Liaison are:
- Improving communication between General Practitioners and Hospital clinical staff with focus on improved continuity of care;
- Increasing understanding of the role of General Practice in the extended health care team;
- Improving relationships between Hospital staff and General Practice; and,
- Providing General Practice input to hospital service planning and implementation.
