Perth Primary Care Network (PPCN) located in Guildford, is a leading not-for-profit organisation in the primary health care industry that focuses on delivering quality health outcomes to local communities by supporting General Practice and the broader primary health care sector.

PPCN operates a well established suicide intervention program, known as ‘ALIVE’.  The program provides intensive three month case management support for at risk of suicide or self harm clients. The program is valued by both Health Professionals and clients alike and has received recognition from leading industry heads for its contribution to suicide prevention.

This position provides assessment, one on one counselling and support to clients who are at risk of suicide or self harm.  This position also provides support and education to GPs in the area of suicide prevention along with enhancing collaborations with other mental healthcare providers to ensure patients receive the highest quality care.

We are seeking to appoint a suitably skilled and experienced individual to provide the highest standard of assessment, counselling and case management to this client group.  The successful applicant will have tertiary qualification in psychology, social work or similar discipline, and have a minimum of two years experience working with clients with mental health and/or psychosocial issues. 

The successful applicant will require highly developed interpersonal and communication skills, and have a sound knowledge and understanding of other community resources.  Additionally, the applicant is required to be registered to an appropriate authority (if applicable), hold membership to their profession’s professional body, and be committed to professional development.

Title General Practitioner – After Hours GP Clinics

PPCN operates the Perth After Hours GP Clinic, located in East Perth and the Swans After Hours GP Clinic, located in Middle Swan.  These Clinics are General Practice Centre’s offering medical services directly to the public and to patients via the overflow from hospital emergency.  

The clinics are fully accredited to Royal Australian College of General Practitioner Standards and are open weeknights, weekends and public holidays.  The Clinics offer a complete range of general practice care, and specialise in emergency and acute care.

We are currently seeking VR GPs to join our committed teams at our Perth and Swans Clinics on various shifts.  These shifts may be filled by one GP or separated if required.   This position offers an attractive rate, including Salary Packaging, and a pleasant and professional working environment.

PPCN operates the mobile medical StreetDoctor service that takes primary healthcare to the marginalised populations of Perth.   

StreetDoctor is an accredited General Practice service that has been successfully providing quality health care to marginalised people and communities since 2001.  StreetDoctor is a mobile, easily accessible, confidential, non-judgmental and non-discriminatory bulk-billing primary health care service.

StreetDoctor attends several different sites around central Perth and the eastern suburbs. The service attends sites that will provide homeless and disadvantaged populations with access to primary health care because access to conventional general practice may not be a possibility for a number of reasons.
