| Job Information | Perth Primary Care Network (PPCN) located in Guildford, is a leading not-for-profit organisation in the primary health care industry that focuses on delivering quality health outcomes to local communities by supporting General Practice and the broader primary health care sector. PPCN operates a well established suicide intervention program, known as ‘ALIVE’. The program provides intensive three month case management support for at risk of suicide or self harm clients. The program is valued by both Health Professionals and clients alike and has received recognition from leading industry heads for its contribution to suicide prevention. This position provides assessment, one on one counselling and support to clients who are at risk of suicide or self harm. This position also provides support and education to GPs in the area of suicide prevention along with enhancing collaborations with other mental healthcare providers to ensure patients receive the highest quality care. We are seeking to appoint a suitably skilled and experienced individual to provide the highest standard of assessment, counselling and case management to this client group. The successful applicant will have tertiary qualification in psychology, social work or similar discipline, and have a minimum of two years experience working with clients with mental health and/or psychosocial issues. The successful applicant will require highly developed interpersonal and communication skills, and have a sound knowledge and understanding of other community resources. Additionally, the applicant is required to be registered to an appropriate authority (if applicable), hold membership to their profession’s professional body, and be committed to professional development.
