Programs and Services

After Hours Clinics

Perth Primary Care Network manages two After Hours GP Clinics – One is located next to the Emergency Department at Swan District Hospital, the other is on Lord Street, across from the Royal Perth Hospital Emergency Department. The clinics are open week nights, weekends, and public holidays.

Aged Care Access Initiative

The Aged Care Access Initiative supports improving access to GP and allied health services for residents of aged care facilities.

GP Liaison

Hospital Liaison General Practitioners provide a vital link between Hospitals, Divisions of General Practice and General Practitioners to improve continuum of patient care through improved communication.

Healthy Directions

Healthy Directions is a single entry point referral service and one-stop shop for all patients with either:

  • Type 2 Diabetes diagnosed less than 5 years; or
  • Type 2 Diabetes with microalbuminuria; or
  • Impaired Glucose Tolerance (IGT); or
  • At high risk of Diabetes (AUSDRISK score 12+)

Immunisation

Perth Primary Care Network is committed to assisting General Practices with improving immunisation coverage rates.  The Network provides practice support via the RDG and ACIR practice visits, relevant education and correspondence concerning updates on immunisation.

MindCare

The MindCare program provides short-term counselling for people who present to their regular GP with a mental health problem, but are unable to access private psychological services.  Through a GP referral, patients are able to access up to 12 sessions of counselling with a psychologist for as little as $5 a session.  The program is not a crisis/emergency or after hours service and there may be a short waiting period.

Nursing in General Practice

Perth Primary Care Network supports the recruitment and retention of nurses in General Practice. The Division provides practice nurses with regular updates in the Weekly Update as well as offering education and networking opportunities.

Quality Use of Medicines

The QUM program is delivered through the National Prescribing Service, a non-profit organisation independent of government and the pharmaceutical industry, to improve the health outcomes for consumers through judicious, safe and effective prescribing.

National Prescribing Service (NPS)

The key roles of the National Prescribing Service are:

  • To provide advice about quality prescribing and quality use of medicines.
  • To develop a national overview for quality prescribing policies, practices and resources.
  • To endorse best practice in prescribing.
  • To provide national leadership to ensure that consistent, independent and objective information about medicines is available for every prescriber.

Street Doctor

Street Doctor is an innovative mobile medical service providing primary health care services to street-based populations in inner city and north-eastern Perth.
