After Hours Clinics

Perth Primary Care Network manages two After Hours GP Clinics, located next to the Emergency Department at Swan District Hospital, the other is on Lord Street, across from the Royal Perth Hospital Emergency Department. The clinics are open weeknights, weekends and public holidays.

Healthy Directions

Healthy Directions is a one-stop shop for all patients with either: 

• Type 2 Diabetes;
• Impaired Glucose Tolerance; or
• At high risk of Diabetes (AUSDRISK score 15+)

• Living Well with Ongoing Health Conditions 

MindCare

The MindCare program provides short-term counselling for people who present to their regular GP with a mental health problem but are unable to access private psychological services.  Through a GP referral, patients are able to access up to 12 sessions of counselling with a psychologist for as little as $5 a session.

Street Doctor

Street Doctor is an innovative mobile medical service providing primary health care services to street-based populations in inner city and north-eastern Perth.
